Certificate and Declaration Questions
- It is assumed that you have declared all existing or ongoing medical conditions to the relevant authority when applying for, renewing, or holding your certificate(s)?
- It is assumed that you have not had a certificate previously refused, revoked, or suspended?
- It is assumed that you have not received a warning regarding your certificate?
- It is assumed that you do not know of a circumstance that may or cause the reason for your certificate(s) to be revoked? (Please note that speeding points up to six do not need to be declared.)
- Do you know of any circumstance that could give rise to a claim under this policy?
